Childcare Liability Waiver

Ignite Cycle LLC (“Ignite Cycle”) offers childcare services to (the “Services”, any individual service, a “Service”) to its Members and registered guests. All parents or authorized persons (“Guardian”) desiring to enroll their child in Ignite Cycle’s childcare services must sign this release and agree to the Ignite Cycle Childcare Policies prior to their child participating in the Services.

            By signing this waiver, Guardian consents to all activities conducted by Ignite Cycle as part of the Services and the participation of their child in the Services. Guardian assumes all risk of injury or harm to their children associated with participation in the Services and agrees to release, indemnify, defend, and forever discharge Ignite Cycle, its staff, employees, officers, and agents from all liability, claims, demands, damages, costs, expenses, actions, and causes of action in respect to injury, loss, damage, or death to any child cause through participation in the Services.

            Guardian agrees to pay all costs, including attorneys’ fees, incurred by any party in defending an investigation, claim or lawsuit brought by Guardian, arising from a child’s participation in the Services.

            Guardian warrants their child is in good health and is capable of participating in the services. In the event Guardian’s child suffers from any life-threatening allergies or other conditions that Ignite Cycle should be aware of, Guardian will disclose such conditions to Ignite below. Guardian authorizes Ignite Cycle to call emergency medical staff or client’s emergency contact in the event of any emergency.

Guardian agrees that their child may be photographed, or video recorded during participation in the Services and that Ignite Cycle may use the images or recording for promotional materials. Guardian may opt out of advertising and promotional considerations of this provision by initialing the Opt-Out option in this agreement or by providing written notice to Ignite Cycle.

Guardian warrants that their child is not suffering from any of the following:

·      An illness, bacterial infection, viral infection, or other condition that a physician determines is infectious or a risk to others;

·      Chicken pox;

·      Vomited within the last twenty-four (24) hours;

·      Abnormally loose stools;

·      Contagious conjunctivitis (pink eye);

·      Unexplained lethargy

·      Lice, ringworm, or scabies;

·      A fever of 99 degree Fahrenheit or higher;

·      An undiagnosed rash;

·      Respiratory illness (coughing); or

·      Runny nose.

Prior to check in for their first Childcare session, guardians are required to fill out a waiver form for each child participating. Please plan to arrive 10 minutes before your scheduled Ride time to complete the form.
